Assistance and Referrals for CareReferral information for health programs and services in San Joaquin
County. Adolescent ProgramsCase management services for teens who are pregnant or parenting a child.
Participants are linked to nutritional, medical, educational and psychological
services as needed. For information or to refer a teen, Black Infant Health ProgramSocial support, case management, referral and assistance into care for
eligible high risk families and infants up to one year of age. For information
or referrals, California Children's Services (CCS)Specialized case management and medical care for children with eligible
physically handicapping conditions; school-based physical and occupational
therapy services for eligible children. Children are referred by health
care providers for eligibility determination, authorization and coordination
of service benefits, and care planning. Child Health and Disability Prevention Program (CHDP)State program providing resources for children (up to age 21) living
in San Joaquin County to receive preventive health care. For information
or referrals to care, CHDP: The Gateway to Health | Newsletters, Provider Resources, Links Child Injury Prevention ProgramChild Passenger Safety Education, low cost car seat distribution and
car seat safety checks. The program provides presentations for the community
on child safety topics. For information on upcoming classes and safety
checks, Childhood Lead Poisoning Prevention ProgramNursing case management for children referred by providers with high
levels of lead in their blood; education services for families, community
groups and health care providers. Public Health Nurse Home VisitsHome visits for high risk families and infants for care referrals and
coordination, developmental assessments and parent education. SIDS (Sudden Infant Death Syndrome) Bereavement SupportSupportive services, referrals, and education for families experiencing
the death of an infant due to SIDS. To refer parents or families, WIC ProgramSupplemental Nutrition Program for Women, Infants and Children
Provides food vouchers, nutrition education breastfeeding support and
referral for health care to eligible women and children. Pregnant, breastfeeding
and postpartum women of all ages, infants and children to age 5 are referred
by health care providers. |
